Changes in Release of Records to GPs/NPs
IMPORTANT PROCESS CHANGE ABOUT RELEASE OF MEDICAL RECORDS TO GPs/NPs
Starting Today Monday, June 3rd, ISH will be asking patients to identify in writing whether they have a family doctor or nurse practitioner, and if so whether they’d like notes from their visit and/or results released to that provider. This change is being made so that we are in compliance with our physicians’ college guidelines, and also to ensure better continuity of care between patients who visit our clinic and their primary care providers.
Please note that we will ONLY release visit notes and/or results to your GP/NP if you **explicitly ask** that we do so, and our nurses and doctors will ask you about this at every visit – we understand that you might want some notes shared with your GP/NP and not others.
